What causes session check failures?

Problem Description:
I've got one user that gets "Session check failures" a lot when posting.

What normally causes these?  He can normally post fine, but like 1 out of 5 times, he gets that error and looses his message.

A lot of the time it's caused by posting after typing for 15+ minutes...

That very well could be the case, since he is usually at work and may be doing 10 other things while posting.

But wouldn't that be a "Session Timeout"?  What is the difference between these two messages?

Also, what determines the length of time before a session times out?  Is that something that can be adjusted?

Should be an option somewhere in php.ini.  Honestly I'm not sure.
