If you mean something like YaBB SE site then all what you need to do is adjusting your template.php to meet your needs.

Goto admin section in your forum, you'll find Edit Your Forum Template copy it to your HTNL editor.
Add whatever you like i.e table , border or classes, then copy it back to your template.

Just make the index page as you like then make your hyperlink target in the same windows.
if your useing any of HTML editor i think opening hyperlinks in the same windows is the default option.

Yabb.nl is just some table's and a who is online block.
Put your forum in a table and the menu's+who ís online in a other table.
simple example:
top

menu and who is onlineforum

code:
[table][tr][td]top[/td][/tr][/table]
[table]
[tr]
[td]menu and who is online[/td]
[td]forum[/td]
[/tr]
[/table]

How to make a whois online block:

Make sure all your files are the most recent 1.5.1 files.  Notably Load.php...
