My server was able to fix it tonight. Here is what he did:

The Package.php was looking for "Packages/modifications.php" which means that the directory placement would have had to have been:
/home/httpd/html/www/community/Sources/Packages

I checked version 1.4.1 and the same error exists there also, because the actual location of the Packages (and modifications.php) directory is here:
/home/httpd/html/www/community/Packages

My solution to this problem was to copy the Packages directory to the first path indicated above.

Thats odd, because the default location for the Packages directory  is at the same level as Sources (or at least that's where it ended up everytime I installed YaBB) but obviously that is not universally true.

So, did you manage to get the mod working ? What do you think?

Yes... it is working just fine now.   Exactly what I was wanting.

I've installed the modpack, without problem. And, I have put a test poll up on the board, but I can not see which members actually voted.  I can only see that 5 people have voted yes, 0 no...  Is there a way I can view which members voted?

Sorry, no. That is a limitation of the YaBB SE poll system.

It is something that I have been asked to look at on the board I wrote this for, but I don't know when I'll get around to it.
