Cuando se quiere cambiar el formato de fecha y hora, hay un pequeño help.  En este help está faltando

%r - time in a.m. and p.m. notation

que es lo que permite setear la hora con el postfijo AM/PM

si %r falta ...

%r es la hora (12 horas con sufijos a.m. /p.m.) a contrario %R (24 horas)

p.s. normalmente se usa %p (no sé si el "hoy mod" funciona con %D %H %r y esas cosas)
